I started with the one from this site:

and was trying to modify it - however we may not do a 504.
I am still working out what we are going to do next year.
That website is a very good start. Just remember as you are planning the 504 to think of all areas of concern. Think about the classroom, lunchtime, and especially field trips. You should try to think of how you would handle things. On the field trips, make sure that you put down that you can always attend all field trips if you can. I left this out of mine and will be making sure that I add it when we revise. There was one on here some where but I cannot remember where. I will try to look for you. After I looked thru it and then looked at the one on Food Allergy Advocate, it seemed they based theirs on that one too. Good luck! Don't give up!
Look at the looking for accepted 504 plans. The one for Jamie is pretty good. I used this as template.
