Hi! I'm a high school student in Connecticut, and I'm doing a research project which looks for a possible link between peanut allergies and immunizations. If you have a child between the age of 6 months and 16 years with a peanut allergy, please help me by filling out this survey:
(If the link doesn't work, just cut and past the 2 lines into your browser. It only takes about 3 minutes.)

I just did it. It really only took less than a minute, and all questions are easy, straightforward, and don't require running to look up any records.
