The last I posted the assistant principal(504 coordinator) asked me for some details I was after for my son's 504...I asked you all, advice was no details until he is eligible, which I did. Still awaiting news, she said probably Wed. In the meantime, I told her I'd work on a 504 outline(start gathering details of what I'd be asking for). I asked the coordinator if the school or district had their own outline(shouldn't they have the info we fill in together, why do I have to provide everything??? this is what she said we would fill out at the meeting.....

""Chanda, below is a copy of the 504 accomodation plan, which will be filled
out during the 504 meeting. I will e-mail you on Wednesday about Jake's
eligibility status. Thanks for your patience.

013fSection 504 Accommodation Plan

Name Date of Meeting

Date of Birth School Grade

(Attach additional pages as necessary.)

Disability Disability symptom Educational need stemming from
symptom Accommodation Whose Responsibility?

(note: there can be more than one need stemming from one
symptom) (expand chart as needed).

1. Describe what appears to be the educational impact of a disability:

2. Describe the basis for the determination of disability (if any):

3. Briefly describe relevant information/data concerning this issue (i.e.,
medical, educational, psychological, etc.); list any assessments used in
evaluation and the date of each."""

Seems a bit vague to me!!! I filled out, in detail the outline from Rhonda, it's pages and pages! The school hands me 3 questions...I don't get it. I have a feeling they are going to be absolutly floored when I approach with the other outline. I wish it weren't so long so i could post it, I'd love to get some advice on it(if I am leaving anything out etc...).

Anyways....I guess the games begin!

Chanda, the form they gave you appears to be an eligibility form as well as a description of accommodations. Here is my suggestion of how to fill it out:
Disability Life-threatening allergies to peanuts, tree nuts and eggs. Asthma. Environmental allergies to pollen, mold, pet dander.
Disability symptom Itching, sneezing, swelling, hives, coughing, difficulty breathing, abdominal pain, vomiting, diarrhea, low blood pressure, light-headedness, loss of consciousness, death.
Educational need stemming from symptom Safe access to all school activities.
Accommodation Goals: [Student] must be protected from contact with peanuts, tree nuts and eggs and accidental ingestion of peanuts, tree nuts or eggs. [Student] must be supervised by an adult trained to administer Epipen at all times. [Student
