The last I posted the assistant principal(504 coordinator) asked me for some details I was after for my son's 504...I asked you all, advice was no details until he is eligible, which I did. Still awaiting news, she said probably Wed. In the meantime, I told her I'd work on a 504 outline(start gathering details of what I'd be asking for). I asked the coordinator if the school or district had their own outline(shouldn't they have the info we fill in together, why do I have to provide everything??? this is what she said we would fill out at the meeting.....

Seems a bit vague to me!!! I filled out, in detail the outline from Rhonda, it's pages and pages! The school hands me 3 questions...I don't get it. I have a feeling they are going to be absolutly floored when I approach with the other outline. I wish it weren't so long so i could post it, I'd love to get some advice on it(if I am leaving anything out etc...).
Anyways....I guess the games begin!

Chanda, the form they gave you appears to be an eligibility form as well as a description of accommodations. Here is my suggestion of how to fill it out:
