Need feedback to develop exciting new tool for those of us who deal with food allergies every day -- specific to restaurants

My name is Paul Antico and I am a father of four children, two of whom have severe food allergies. Having worked hard at keeping my children safe while accommodating their needs, I certainly understand the difficulties of dealing with food allergies each and every day.

It is based on these experiences, and the experiences of many others I know who also deal with food allergies, that I have come up with an idea that I believe may be attractive to all of us who are affected by this issue. However, to fully develop this idea, I need feedback from more people who deal with food allergies every day (whether their own allergies or someone they help care for). I am writing this note in the hope that you might consider aiding me in this endeavor.

If you are willing to help, I would simply ask you to participate in a quick survey. The survey is made up of 21 multiple choice questions and should only take 5-10 minutes. During the course of the survey, you will learn more about my idea and will have a chance to leave further feedback at the end, should you so choose.

Hi Paul, I posted a blog post on Best Allergy Sites about your survey and also posted the link on the Food Allergy Support forums. And I of course took the survey.
